It can be difficult to allow someone into your home to assist with your personal care needs, medication prompts and/or meal preparation. The resolute staff at Outreach, will work with you and in collaboration with your Support Coordinator and allied care team to assist with your needs and in doing so…develop a trusting relationship between you, your allied team and all our Outreach Community Care staff members.
Knowing all too well how busy life can be for our extended family members, whom in a lot of occasions, also take on the role of the carer for their loved ones. As much as they feel they must take on this role, it can be a very overwhelming, time consuming, making the family member time poor to disengage and take time for their own wellbeing and mental health. Over time being the constant carer/care giver, makes the family member feel resentment to towards the family member requiring their assistance, tired and in a lot of cases, guilt. With Outreach Community Care at your service, we can assist with all the participants needs, whilst giving a much deserved and needed break to the family member/s, knowing their loved one is in excellent qualified hands.
Outreach Community Care will work with you, support you, and encourage you to stay at home and strengthen your independence. This can demonstrated in a number of diverse ways, for example: Personal Care, Domestic Assistance, shopping, cooking and general community participation. Trust and communication, between Outreach Community Care staff and Participants, are essential tools and are at the core of everything we do, throughout every facet in your pursuit of reaching your goals and being satisfied with your achievements.
We have all been in that saddening time, when a loved one is close to saying their final goodbyes to a family member. It is at this time, where Outreach Community Care’s highly trained and compassionate staff can be involved in every stage of your loved one’s needs, whilst comforting and supporting the family member who will eventually be left behind. Our caring and nurturing staff will deliver exemplary care and attention with your loved one and will make every attempt to make your loved one as comfortable, supported and will be attentive as if it were one of their own family members. It is another of Outreach Community Care’s core belief’s that every Participant/loved one to be treated with the utmost respect and to see that their wishes are met to the letter. At Outreach Community Care, we have a dedicated team of nursing staff who can and will attend to your wound dressing requirements post operatively, as directed by your allied health care team.

Although it currently does not provide wheelchair access vehicles, the company is actively working towards improving its services and plans to introduce a wheelchair-capable vehicle in the near future. The management understands the importance of accessibility and is committed to providing safe and comfortable transportation solutions for individuals with disabilities. The company is dedicated to enhancing the quality of life for its clients and is continuously exploring ways to improve its services to meet the needs of all its clients, including those with disabilities.
